Health and care organisations in Cornwall and the Isles of Scilly have published their proposal for service transformation over the next five years.

Preventing ill health and more care at home are at the heart of health and social care plans for Cornwall and the Isles of Scilly.

Cornwall Council have more information on their website and are running an online survey, please visit www.cornwall.gov.uk/shapethefuture

They are also running community events in January 2017, dates and locations can be found on the website link above.
